Borrowing Form

Thank you for your interest in borrowing scores from the Washington Men's Camerata's national library of men's choral music. Please read the terms and conditions of this loan agreement submit. Upon receipt of your request, the Camerata will promptly verify that your requested scores are available and reply.

Requester Information

The undersigned agrees by checking the consent box below:

  1. Payment: The Borrower will pay to The Lender (Washington Men's Camerata) a nominal handling fee (typically less than $40) for processing this request. The handling fee is waived for requests by choruses within the Washington, DC Metropolitan Area. Otherwise, The Lender will contact The Borrower to discuss payment for shipment.
  2. Duration of Loan: The requested scores must be returned to The Lender on or before the date selected on this form.
  3. Returning Scores: Returned scores must be shipped to The Lender by hand delivery, prepaid delivery by UPS, or first class USPS. (Note: UPS is the preferred method of shipment.)
    If scores are not delivered by the due date, they will be deemed lost and subject to the Lost Score Fee.

  4. Program Credit: If a program is issued in conjunction with a performance of any of the borrowed works, the program page must contain the following statement in at least 12-point type: "Scores for this performance have been provided by the Washington Men's Camerata's National Library of Men's Choral Music." A copy of the program(s) shall be sent to The Lender at the address listed above in the Returning Scores section above along with the returned scores.
  5. Lost or Damaged Scores: All borrowed scores will be evaluated upon return to WMC. The Borrower agrees to replace or reimburse WMC for any lost or damaged scores. Replacement value is determined by WMC.
  6. Markings: All new markings must be made in pencil and erased prior to return.
  7. Performance Rights, Recordings & Clearances: Organizations wishing to perform or record borrowed music that is under copyright MUST secure performing or recording rights. The Borrower affirms understanding and agrees to secure such rights and clearances and assumes full responsibility for the payment of any royalties or fees for performances or recordings.
  8. Reciprocity: The Borrower agrees, to the extent possible, to reciprocate the courtesy of lending music at nominal cost to the Washington Men's Camerata and other choruses.
